Passports

Passport services (new applications, renewals, official/diplomatic, lost/stolen, visa pages, and name changes) and citizenship claims are available in the Consular Section of the U.S. Embassy by appointment.  All U.S. passport applications are processed in the United States, which takes at least two weeks.  Please plan your travel accordingly.

The consular section does not offer photo services.

  • Additional Visa Pages
     

    If your passport’s visa pages are becoming full, but your passport is still valid and in good condition, you may apply for additional pages to be added to your passport.

    NOTE:  If applying for children under 16, a parent should sign the application form on behalf of the child. At the time of the application, the signing parent should also submit his or her passport. The personal appearance of the minor and two-parent consent are not required.

    Please submit:

    • valid ten-year or five-year passport
    • completed form DS-4085

    There is an $82 fee for this service, payable in U.S. dollars or by credit card.

  • First Time Passport
     

    Adults (Age 16 And Older)

    Please submit:

    • original proof of citizenship (e.g. U.S. birth certificate, Consular Report of Birth Abroad, or Certificate of Naturalization)
    • one valid form of photo identification (e.g. foreign passport, driver's license, military ID, etc.)
    • one passport photograph (PDF 1.03 MB)
    • completed form DS-11

    Children Under 16

    Change in Passports for Children:

    As of February 1, 2008, the two-parent consent requirement for all passport services was extended to include all children under age 16.  Either both parents must be present to provide signatures at the time of passport application, or in the event one parent cannot attend, he or she must send a notarized statement of consent and valid identification.  Alternately, the applying parent may present a death certificate or sole custody order for the child.

    Please submit:

    • original proof of citizenship (e.g. U.S. birth certificate, Consular Report of Birth Abroad, or Certificate of Citizenship)
    • one valid form of photo identification (e.g. foreign passport, driver's license, military ID, etc.)
    • one passport photograph (PDF 1.03 MB)
    • completed form DS-11

    All first time applicants must appear in person.

  • Lost or Stolen Passports
     

    In order to replace a lost or stolen passport, you must apply in person and submit:

    • original proof of citizenship (e.g. U.S. birth certificate, Consular Report of Birth Abroad, or Certificate of Naturalization)
    • one valid form of photo identification (e.g. foreign passport, driver's license, military ID, etc.)
    • one passport photograph (PDF 1.03 MB)
    • completed form DS-11 and form DS-64

  • Military Passport Acceptance Agent Program
     

    The U.S. Embassy in Baghdad partnered with the Department of Defense to establish a Military Passport Acceptance Agent Program in Iraq.  Under this program, U.S. soldiers may apply for their U.S. passports at military legal assistance offices on a number of bases throughout Iraq.  After processing at the Embassy and shipment from the United States, the American Citizen Services Unit will arrange delivery of the new passports to the appropriate agents.  The soldiers may then return to the passport agent to retrieve their new passports.  As a result, the soldiers can avoid a specific trip into Baghdad to apply at the Embassy location in the International Zone.  This program provides a convenient alternative and shortens the soldiers’ time away from duty.

    The American Citizen Services Unit currently partners with agents in at the following base locations:  Adder, Al Asad, Balad, Delta, Diamondback, Echo, Kalsu, Ramadi, Speicher, Taji, Victory (and Liberty), Warhorse, Warrior, and Basrah.

  • Minors
     

    Applying for Passports for U.S. Citizens Under the Age of 16

    Change in Passports for Children:

    As of February 1, 2008, the two-parent consent requirement for all passport services was extended to include all children under age 16.  Either both parents must be present to provide signatures at the time of passport application, or in the event one parent cannot attend, he or she must send a notarized statement of consent and valid identification.  Alternately, the applying parent may present a death certificate or sole custody order for the child.

  • Name Changes
     

    If you have changed your name, you may apply to have this change reflected in your passport.  Please note that as of September 26, 2005, U.S. passports will no longer be amended to change items appearing on the data page.  You will therefore apply for a new passport in your new name.

    If the change is within one year after issuance, please apply and submit:

    • current passport
    • completed form DS-5504
    • one passport photograph (PDF 1.03 MB)
    • legal documentation of name change (e.g. original marriage certificate, divorce decree, or court order)

    There is no fee charged for this service.

    If the change occurs more than one year after issuance, please follow the procedure for passport renewal and include documentation of name change (e.g. original marriage certificate, divorce decree or court order).
     

  • Official and Diplomatic Passports
     

    Official and diplomatic passports are issued only in Washington, D.C. by the Special Issuance Agency (SIA) to employees of the U.S. government traveling or on assignment abroad on official business.  However, the Consular Section can accept applications and forward them to Washington, D.C. for processing.

    To apply for an official or diplomatic passport, you must present:

    • either a completed but unsigned passport application DS-11 (if this is the first time you have applied for any kind of passport) or a completed DS-82  (if you have a tourist passport or are renewing your diplomatic or official passport)
    • one passport photograph (PDF 1.03 MB)
    • original proof of citizenship (e.g. U.S. birth certificate, Consular Report of Birth Abroad, or Certificate of Citizenship)
    • one valid form of photo identification (e.g. foreign passport, driver's license, military ID, etc.)
    • typed and signed DD-1056 (if you are with the Department of Defense)
    • a copy of your orders from your Department or Agency and
    • a memorandum from your headquarters office outlining the need for an official or diplomatic passport (if you are from a Department or Agency other than DoD)
    • a copy of your travel authorization and your contract (for contractors)

    There is no fee for an official or diplomatic passport.

    The current turnaround time from the SIA for official or diplomatic passport is a minimum of two months.  When planning on renewing your official or diplomatic passport, also note that some foreign governments require a minimum of 6 months validity remaining before your passport expiration date for certain visas.

  • Passport Card
     

    The passport card facilitates entry and expedites document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda.  The card may not be used to travel by air.  It carries the rights and privileges of the U.S. passport book and will be adjudicated to the exact same standards.

    The Department of State is issuing passport card in response to the needs of border resident communities for a less expensive and more portable alternative to the traditional passport book.  The card has the same validity period as a passport book: 10 years for an adult, five for children 15 and younger.  For adults who already have a passport book, they may apply for the card as a passport renewal and pay only $30.  First-time applicants will pay $55 for adult cards and $40 for children.   

    To facilitate the frequent travel of U.S. citizens living in border communities and to meet DHS’s operational needs at land borders, the passport card contains a vicinity-read radio frequency identification (RFID) chip.  This chip will link the card to a stored record in secure government databases. There will be no personal information written to the RFID chip itself.

  • Passport Renewals
     

    Adults (age 16 and older)

    If your most recent passport was issued within the last 15 years and you were age 16 or over at the time of issuance, please submit:

    • your most recent U.S. passport
    • one passport photograph (PDF 1.03 MB)
    • completed and signed form DS-82.  

    Children under 16
