Replacing a Lost or Stolen Passport

To replace a lost or stolen you must present the following:

  • A completed but unsigned passport application DS-11;
  • A completed but unsigned affidavit of lost or stolen passport, DS-64;
  • Two recent identical color photographs (see Photographic Requirements);
  • A copy of a report from either the Iraqi police or the U.S. military if your passport was stolen;
  • An application fee of $100 payable in U.S. Dollars, cash only.

The consular officer is required to verify both your citizenship and identity. Bring with you all documents in your possession that would help to prove both your citizenship and your identity. This process may take an extended period of time if such documentation is not available.

Once the consular officer verifies your U.S. citizenship and identity, your passport application will be forwarded to the U.S. for issuance of a ten-year, photo-digitized passport. Allow up to one month for passport processing.
