Changes / Corrections to Current Valid Passports

As of September 26, 2005, the Department of State no longer amends passports. For security reasons, it is required that passports with errors or changes be replaced.

If it has been MORE than one year since the passport was issued, you must submit Form DS-82 (see Passport Renewals) and pay the applicable fees. No fees are required when the cause is a printing error by Passport Services.

If there was a change in is your identifying information (e.g., name change due to marriage or by court order), or you discovered a Passport Services’ printing error WITHIN the first year from the date of issue, we will replace the passport for free.

You must present:

  • Re-Application Form DS-5504;
  • The passport to be replaced;
  • The documentation required for the change (see Instruction Page 2 of the form);
  • Two recent identical color photographs (see Photograph Requirements).

Once the U.S. consular officer verifies your U.S. citizenship and identity, your passport application will be forwarded to the U.S. for issuance of a 10-year, photo-digitized passport. Allow up to one month for passport processing.
