First Time Passports

If you are applying for your first U.S. passport, your last U.S. passport was issued more than 15 years ago, or your passport was lost/stolen, you must present:

  • A completed but unsigned passport application DS-11;
  • Proof of U.S. citizenship;
  • Proof of identity;
  • Two recent, identical color photographs; and
  • An application fee of $100, payable in U.S. Dollars, cash only.

Proof of Citizenship

For applicants born in the United States, a certified birth certificate from the state vital statistics office in the state in which you were born is required. Hospital-issued certificates will not be accepted. If you need to request your birth certificate, please visit the National Center for Health Statistics Web site for contact information for the state registrars of vital statistics.

If you are a naturalized U.S. citizen, you must present an original Naturalization Certificate.

If you are claiming U.S. citizenship through birth to American citizen parents but were born outside the U.S., you must present an original Consular Report of Birth Abroad.

Proof of Identity

Current identifying documents containing your name and signature along with a physical description or photograph (e.g., previous passport, Certificate of Naturalization, driver's license, etc.)

Photographic Requirements

Note that the photographs must be 2" x 2", have a plain, white background with full face forward, and head size approximately 1 to 1-3/4". Passport photograph requirements are strictly enforced. See Photographic Requirements for more information.

Once your application is approved by the U.S. consular officer, it will be forwarded to the U.S. National Passport Center for issuance of a 10-year, photo-digitized passport. Please allow up to one month for passport processing.
