Be sure you have the completed DS-156 including the third page with the large barcode, completed DS-157, completed Supplement to DS-157, and completed DS-158. You will submit all of these forms, along with a passport picture, your original passport, a copy of the data pages of your passport, and a copy of your Civil ID (“Hawia Ahwal Al-Madania”) to the Consular Section.
