NIV Electronic Forms

  • EVAF link for DS-156 Form

    This form is required for all applicants, and must be filled out completely online. After pressing "Continue", the applicant needs to print out and sign the PDF form.
  • DS-157 Form

    The DS-157 is required for all applicants. This form must be filled out by hand BEFORE the day of the interview.
  • DS-158 Form

    This form is required for all applicants applying for F,J,& M visas. This form must be filled out by hand BEFORE the day of the interview.
  • Iraq supplemental form

    The Iraq supplemental instruction is required for all applicants in Iraq. The questions on the form must be filled out on the DS-157 BEFORE the day of the interview.
